Solon-based Chronic Care Management, a health-tech startup developing a platform to help physicians navigate the complexities of care management and improve  reimbursement, raised $2.55 million of angel funding from undisclosed investors, according to PitchBook.

The company previously raised $4.99 million in a Series A funding round from Chicago-based B.C. Ziegler & Co. and other unidentified investors, PitchBook reported.

Chronic Care Management's platform reports on key performance indicators to show key metrics on patients, according to the company's website. It said that in 2018 the company served more than 26,000 patients and avoided 6,100 hospitalizations
